I just learned that I was born in your hospital on 4-21-1982 my name is Richard Ray Kincaid Jr. I was born to Mary Kincaid and Richard Ray Kincaid Sr. I do not know my mother's maiden name. I need my birth certificate in order to get a valid Id. Please contact me and tell me how and what I have to do in order to get my birth certificate from you. I moved to missouri and I have to have a valid ID. I cannot get one without my birthcertificate. I cannot get my birthcertificate without my mother's maiden name (WHICH I DO NOT KNOW) I only know her married name.
